Before your first Physical Therapy Appointment:

Print out and complete intake forms:

This is not necessary. Our physical therapists will provide our intake forms upon arrival. However, filling out our patient intake forms at home will reduce your wait time before receiving hands on care by one of our physical therapists. Below in the "downloads" section is a  link to our patient intake forms.  



Bring any important medical forms:

This includes:

  • Doctor's Referrals/ Prescriptions
  • Medication List
  • Imaging Reports (X-rays, MRIs, CT scans)


These items can help our Physical Therapists better understand your medical needs and help with rehabilitation goals. 

Bring Your Insurance Card:

A copy of your insurance card and a valid form of identification (driver's license) will be made upon arrival. These will be used for billing purposes and insurance authorization. 


Our staff will contact your insurance provider prior to your first visit in order to determine you insurance benefits. However, it is always a good idea to know your insurance benefits including: deductibles, copays, responsibilities etc.

What to wear:

Typically our patients will wear loose, flowing clothing. You will be working out after all! Our physical therapists may need to inspect the skin/ post operative incisions in some cases. In addition, most patients will receive some combination of manual therapies, which typically are performed on the skin's surface. So do not be afraid to wear sweat pants, shorts or T-shirts.  

Other:

Please tell your Physical Therapist of any allergies you may have, as some of our equipment may contain latex. 

Inform your physical therapist of any life-saving rapid response medication you may use including:

  • Inhaler
  • Nitroglycerin
  • Epipen
